I AM 3RD Safe Sanctuary Form (PDF)
All new coaches are required to complete and turn in this form to I AM 3RD before the first practice. Current coaches, you must submit a new form every year to be able to continue coaching I AM 3RD players. If you're unsure of whether or not you need to submit a new form, please contact I AM 3RD.
